Comprehensive care starts with a comprehensive understanding of you
Most people do not need more information. They need clarity, prioritization, and a physician who can translate data into durable action. Enrollment is that partnership — ongoing guidance, medical care, interpretation, accountability, and refinement, not isolated visits.
Enrollment begins with understanding where you are today, then builds an ongoing strategy around your data, goals, and evolving health.

How it works
From first call to ongoing care
No commitment up front. Membership follows a conversation, a measured baseline, and a plan built on your data.
01
Discovery call
A free 15-minute conversation about your goals and whether the practice is the right fit.
02
Baseline Assessment
One appointment: physician consultation, VO₂ max and threshold testing, and body composition.
03
Personal plan
Physician interpretation of your results, delivered as a written strategy you can act on.
04
Ongoing membership
A continuing physician partnership — guidance, accountability, and reassessment over time.
From initial evaluation to ongoing care
Enrollment begins with a comprehensive evaluation to understand your health, physiology, goals, and priorities. From there, we build, implement, and continuously refine your plan as your needs evolve.
Comprehensive evaluation
We start with your medical history, goals, current health, and objective measurement to establish where you are today.
Physician interpretation
Your results are integrated into a coherent picture of your health, risks, physiology, and opportunities for improvement.
Personalized strategy
We identify what matters most and build a practical plan around your goals, preferences, and life.
Longitudinal care
Ongoing access, treatment, guidance, accountability, and reassessment allow the plan to evolve with you.
Enrollment
Partnership built around you
Care built around your health, physiology, and goals — not a generic protocol.

Nothing is purchased on this site. Already completed a Baseline Assessment? Your results carry straight into membership. Not sure which fits? That's exactly what the discovery call is for.
Enrollment, answered
What does enrollment include?
Enrollment allows us to know you as a person - your health status, history, values and goals. It is what makes ongoing, unhurried, data-driven care possible: more time with your physician, prevention-first planning, coordination of outside records and referrals, and longitudinal tracking of your progress. Specific benefits, visit frequency, and pricing depend on the partnership you choose.
Do you accept insurance?
TMD Health is a self-pay practice and does not participate with insurance plans. Superbills are available on request for you to submit to your insurer, though reimbursement cannot be guaranteed. To avoid surprises, a Good Faith Estimate is provided. Independent labs and imaging centers bill separately.
Does TMD Health replace my primary care doctor?
No. TMD Health focuses on prevention, performance, and longevity, and is not intended to be your sole primary care provider. It does not provide hospital, emergency, urgent, or 24-hour on-call care — we recommend you keep a relationship with a primary care physician and appropriate specialists. In an emergency, call 911.
How do I communicate with the practice?
Anything involving personal health information goes through the secure patient portal, the office phone during business hours, or direct discussion with your physician. Email is fine for anything non-clinical. The practice aims to respond within one business day — and electronic messages are never the right channel for urgent concerns.
What if I travel outside New Jersey?
If you'll be away for more than two consecutive weeks, let the practice know in advance — with notice, continuity planning is often possible. Because care can't legally be provided while you're physically outside New Jersey, plan for local access to medical care at your destination.
