Most people piece together their approach to health without a clear understanding of their underlying physiology. The Baseline Assessment combines objective measurement with physician interpretation to identify your priorities and build a strategy around what matters most.

- **Body composition & metabolic health** — Beyond the scale: lean mass, fat mass, and the metabolic markers that describe how you actually carry weight.
- **Strength, power & neuromuscular** — Grip strength, power, and movement quality — measured, not estimated.

2. **Intake** — Complete intake forms securely in the practice's medical record system.
3. **Test** — Testing and your physician consultation happen in one visit in Red Bank.
4. **Understand** — Your results, interpreted — with a written strategic summary and recommendations.
5. **Continue** — Evaluate membership options for ongoing implementation, tracking, and refinement.

**Pricing**

## Comprehensive Baseline Assessment — $1,250

Covers everything above: testing, consultation, interpretation, and your written strategy. Additional labs and imaging, when indicated, are billed separately by independent facilities. TMD Health is a self-pay practice; superbills are available on request.

## Who the Baseline Assessment is for

It is an entry point into the practice, geared toward two people: someone who wants a comprehensive evaluation and an actionable plan but isn't ready to commit to ongoing care, and someone who wants an objective understanding of their physiology but intends to manage the strategy themselves.

## How it relates to membership

The Baseline Assessment is not required before becoming a member — and members do not pay for it separately. If you join, a comprehensive baseline evaluation is part of your initial intake. If you complete the assessment first and decide to become a member later, your results serve as the foundation for ongoing care.

### Who is the Baseline Assessment for?

Adults focused on longevity and prevention, endurance or strength athletes, people reducing cardiovascular or metabolic risk, anyone returning to fitness, and anyone who wants physician-guided interpretation of their own data. You do not need to be an athlete — you need to want an objective starting point.

### What happens after the Baseline Assessment?

You receive a written strategic summary and an extended physician consultation covering your strengths, limitations, and highest-leverage opportunities. From there, most people continue into a membership so the plan is implemented, measured, and refined over time — but there is no obligation to.

### Do you accept insurance?

TMD Health is a self-pay practice and does not participate with insurance plans. Superbills are available on request for you to submit to your insurer, though reimbursement cannot be guaranteed. To avoid surprises, a Good Faith Estimate is provided whenever possible. Independent labs and imaging centers bill separately.

### Do you offer telemedicine?

Yes, when clinically appropriate. New Jersey law requires that patients be physically located in New Jersey during telemedicine visits, and some encounter types need to happen in person.
